However, many patients either fail to respond or relapse after initial treatment. This resistance is increasingly understood to arise not only from changes within tumor cells, but also from therapy-induced immune dysfunction, including impaired natural killer cell activity, T-cell exhaustion, expansion of suppressive myeloid cells, and altered CD38 expression on both malignant and healthy immune cells.
